Removing tethering network setting

Today I downloaded the unauthorized version of the AT&T US tethering thingy, and now I want it off my phone. A total restore did not get rid of it, nor did "reset network settings" I tried to "reset all settings" but do not know my passcode (?) Can anyone help me?

I tried my settings passcode one more time, got in, and did a "reset all settings" that got rid of the tethering thingy.

if you do a restore and set it up as a new iphone it will be gone. I just did this myself.
